No. Submitting this form creates a transportation request. Secure Med Trans must review the requested transportation type, schedule, route and provider availability before transportation can be confirmed.
How far in advance should I request medical transportation?
Submitting transportation requests as early as possible gives more time to review the trip and transportation availability. Short-notice requests may also be submitted, but availability cannot be guaranteed.
Can I request transportation anywhere in the United States?
Secure Med Trans accepts transportation coordination requests across the United States. Availability varies according to geography, trip requirements, scheduling and available transportation providers.
Can I request recurring medical transportation?
Yes. Recurring transportation requests can be submitted for dialysis, therapy, rehabilitation, treatment programs and other repeated healthcare appointments.
Can healthcare facilities submit transportation requests?
Yes. Hospitals, nursing facilities, rehabilitation centers, case managers, discharge planners and other healthcare organizations may submit transportation requests through Secure Med Trans.
What transportation types can I request?
Requests may include ambulatory transportation, wheelchair transportation, door-to-door medical transportation, dialysis transportation, hospital discharge transportation, senior transportation and other appropriate non-emergency medical transportation services.
Should I include medical records or diagnosis information?
No. Please do not include medical records, diagnoses, Social Security numbers, insurance identification numbers or other unnecessary sensitive medical information in the general reservation form.
